What do I need to apply for a passport?

GENERAL REQUIREMENTS:

U.S. Birth Certificate issued by the city, county, or state of birth
Certificate of Naturalization
U.S. Passport
IMPORTANT: When submitting a passport for a minor, a copy of the birth certificate must also be submitted
Please note: a Social Security card is not considered evidence of citizenship for passport purposes.
